Eventology Homeschool Prom 2025

I, the legal guardian/parent, or oneself if eighteen and over, understand by purchasing my student/child(ren)/guest or my own, if over eighteen,  an event ticket to the Eventology Homeschool Prom 2025, I’m agreeing to abide by all rules, procedures, and this waiver. I also, hereby grant permission for my student/child(ren)/guest/myself to participate in the Eventology Homeschool Prom for homeschool students ages 14-20 and their guests, which is scheduled for April 12, 2025 from 7:00 pm. to 11:00 pm. This event will be held at the Forum River Center, located at 301 Tribune St, Rome, GA 30161. I understand that it is Eventology's desire to provide an enjoyable and safe environment for the students at the Prom.

I understand that my student’s/guest’s, or oneself, participation in the Event is entirely voluntary and that my student’s/guest’s, oneself, participation potentially involves risks and obligations that are impossible to predict but which are beyond the scope of those normally associated with traditional school functions. These risks include, but are not limited to, the risk of loss or damage to personal property and the risk of sickness, personal injury or death while participating in the Event. I understand the risk and voluntarily assume all risks associated with attending prom.  In the event of injury, I do hereby consent to any emergency medical treatment rendered to my student/guest, or oneself, under the supervision of professional health care providers.

I hereby understand that if I (if over 18 and purchasing for yourself) or my student/guest has any allergies that Eventology Homeschool Events Corp should be aware of, I must email these allergies to samantha@eventologyhomeschoolevents.org, immediately following the purchase of  prom tickets. 

Please note the following:

• Eventology Homeschool Prom has a ZERO tolerance policy for alcohol, drugs, smoking (includes vaping), or misconduct. If you are

found to violate these policies, you will be escorted from the dance with a phone call to your parents. A refund WILL NOT be issued.

• NO weapons, including any gun or knife, of any sort, any kind, in any form, are allowed on the premises for any reason whatsoever. Any Violations of the above rules will result in being banned from any of our events!
• Outside food and drink are not allowed.
• Only clutch style purses  will be permitted inside the venue.

 I give permission for Eventology personnel or authorized volunteers to search any personal belongings that are in my student’s/ guest’s possession upon entering the facility, in addition to being scanned by metal detectors. 

I understand that my student/child(ren)/guest will only be supervised while inside the venue and only during the hours of 7:00 pm-11:00 pm. Additionally, I understand that if my child voluntarily exits/leaves the homeschool prom and/or venue, it is not the responsibility of Eventology personal or chaperones/volunteers to continue supervising. 

I give permission for Eventology to use the photograph(s) for presentation under any legal condition, including but not limited to publicity, copyright purposes, illustration, advertising, and web content. I hereby waive any right of inspection or approval, and I have no right or claim to any royalty, payment, or other compensation arising out of or in connection with any use hereunder. 

I consent to the use of photographs and videos taken during the event for promotional or educational purposes, including on the website, social media, and printed materials.

I hereby release, discharge, and hold harmless Eventology Homeschool Events Corp,, it’s Directors, officers, agents, employees, and authorized/affiliated volunteers from any and all liability, liens, claims, demands, actions, personal injury or cases of action, whatsoever arising from my own or student’s/guest’s participation in any aspect of the Eventology Homeschool Prom, regardless of whether such claims arise from negligence or other factors. 

As the parent or legal guardian of the participant, I have read, understand, and agree to the terms of this waiver and release of liability on behalf of my child. 

As a guest over the age of 18, I hereby confirm that I have read, understand, and agree to the terms of this waiver and release of liability.
